Transition Support
CFWA have recently developed a ‘transition support program’ for young adults transitioning from Princess Margaret Hospital (PMH) to adult care at Sir Charles Gairdner Hospital (SCGH).
The pack includes a back pack/hand bag, food vouchers, Transperth information, a diary, as well as a range of useful resources including budgeting tips for young adults, advice on gaining financial assistance, tips for taking charge of your own life and hot spots for food and recreation around SCGH.
