Member Assistance Form

Individuals living with cystic fibrosis and their families can access full and partial subsidies for the care and treatment of cystic fibrosis. This may include for example vitamins, medical equipment, sport and fitness apparel, equipment or fees or nutritional supplements.

Accessing Members Assistance

  1. You need to be a current member to apply for members assistance. If you do not have a current membership, submit your membership application first. Membership is $0, but requires annual completion of the Membership Survey.

  2. Members assistance is capped at $500 per year, per child (under 18yrs) and $1,000 per current adult member. Once this has been achieved further assistance will be considered by the Management Committee at the monthly Committee Meeting (held the second Wednesday of each month), but is not guaranteed. Vitamins and nebulisers are consider outside of the cap.

  3. Receipts with the supplier contacts and ABN must be provided and attached. Where an individual is unable to cover the cost you may discuss with CFACT paying direct to the supplier on your behalf.

  4. Items that are not covered include, but not equal too; travelling for medical interventions (please note you can apply for IPTAS), assistance for medications, home modifications, hosehold bills, schools fees.

  5. In some cases further information may be sort by your or your CF Clinic team regarding your request. In this instance a representative from CFACT will be in contact with you.

If you have recently changed your address please email us your updated information. Items will be shipped to the address confirmed on your membership.
